SECTION 1: SERVICE AUTHORIZATION

I authorize the salon and my stylist to perform the services discussed during my consultation. I understand that my stylist will assess my hair's condition, review my hair history, and explain the recommended service before beginning.

I understand that I may ask questions, decline a recommended service, or withdraw my consent before or during the service.

SECTION 2: CHEMICAL SERVICE RISKS & INFORMED CONSENT

I understand that hair color, lighteners, toners, glosses, relaxers, permanent waves, smoothing treatments, and other chemical services may involve risks, including but not limited to:

• Hair dryness, brittleness, breakage, or changes in texture.

• Unexpected color results, uneven color, unwanted warmth, or color that differs from inspiration photos.

• Scalp irritation, itching, redness, burning, or chemical injury.

• Allergic reactions, including potentially serious reactions.

• Damage to previously processed, fragile, or compromised hair.

• Results that require additional appointments, maintenance, or corrective services.

I understand that the extent of these risks depends on my hair's condition, previous treatments, individual sensitivities, the products used, and other factors.

I acknowledge that my stylist cannot guarantee an exact color, texture, level of lightness, or result. My stylist may recommend a strand test, an appropriate allergy test, a modified service, multiple appointments, or postponing or declining the service when appropriate.

I understand that chemical services will be performed in accordance with applicable requirements and the manufacturer's instructions.

SECTION 3: CLIENT DISCLOSURE & HAIR HISTORY

I agree to provide complete and accurate information about my hair and scalp, including:

• Previous professional color, bleach, highlights, and color corrections.

• Box dye, henna, metallic dyes, or other at-home color products.

• Previous perms, relaxers, Brazilian Blowouts, or smoothing treatments.

• Known allergies, sensitivities, scalp irritation, or previous reactions to hair products.

• Any other information that could affect the safety or outcome of the service.

I understand that incomplete or inaccurate information may affect my results and increase the risk of an adverse reaction or hair damage.

I understand that my stylist may decline or modify a service if my hair or scalp is not suitable for the requested treatment.

SECTION 4: ALLERGY TESTING & SAFETY

I understand that a negative allergy test does not guarantee that an allergic reaction will not occur. I agree to follow all applicable product-specific testing instructions and salon safety requirements.

I agree to notify my stylist immediately if I experience burning, severe itching, swelling, difficulty breathing, or other unusual symptoms during a service.

I understand that my stylist may immediately stop the service and take appropriate safety measures if a reaction, injury, or other safety concern occurs.

I acknowledge that I must inform my stylist of any new allergy or reaction history before future appointments.

SECTION 5: BRAZILIAN BLOWOUT & SMOOTHING TREATMENTS

If applicable, I understand that smoothing treatments may involve chemical products and heat styling, and that results and longevity vary according to hair type, product, technique, and aftercare.

I understand that potential risks may include irritation, changes in hair texture, dryness, breakage, and reactions to product ingredients or treatment fumes.

Before treatment, I agree to review the specific product being used, its applicable safety information, and any relevant restrictions or aftercare instructions with my stylist.

I understand that my stylist may decline the service if the product, my hair condition, or my circumstances make treatment inappropriate.

SECTION 6: RESULTS, PRICING & ADDITIONAL SERVICES

I understand that my stylist will make reasonable efforts to achieve the results discussed during my consultation, but exact results cannot be guaranteed.

I understand that inspiration photos are examples and may not be achievable on my hair in a single appointment.

If additional time, product, or services become necessary, my stylist will explain the proposed changes and any additional charges and obtain my approval before proceeding.

I understand that corrective color, additional lightening, or major transformations may require multiple appointments and additional fees.

SECTION 7: NO-REFUND POLICY

Payment is due according to the salon's stated payment terms.

Because salon services involve customized professional labor, time, and products, all completed services are nonrefundable, except where a refund is required by applicable law or otherwise required by the salon's legal obligations.

Dissatisfaction with a result does not automatically entitle a client to a monetary refund. However, the salon encourages clients to communicate concerns promptly so that the salon can assess the situation and discuss an appropriate resolution.

If you have a concern about your service, please contact the salon within seven (7) calendar days of your appointment, when reasonably possible. The salon may request photographs or an in-person assessment to understand the concern.

Where appropriate, the salon may offer a consultation, adjustment, or corrective service at the salon's discretion, subject to the condition of the hair, professional judgment, and applicable law. Any complimentary correction must be agreed upon in advance.

Additional services requested to change the original agreed-upon result, or services made necessary by undisclosed hair history or subsequent at-home treatments, may incur additional charges with the client's approval.

Nothing in this policy limits any consumer rights or remedies that cannot legally be waived.

SECTION 8: CANCELLATIONS & APPOINTMENT CHANGES

I understand that the salon may maintain separate policies regarding deposits, cancellations, late arrivals, and missed appointments. Any applicable fees and requirements will be communicated to me before they apply.

I acknowledge that I am responsible for reviewing and following the salon's published appointment policies.

SECTION 9: ACKNOWLEDGMENT & ASSUMPTION OF INHERENT RISKS

I acknowledge that I have had the opportunity to discuss the proposed service, its potential risks, available alternatives, expected results, and aftercare with my stylist.

I understand that some risks are inherent in chemical hair services and that no stylist can guarantee a particular result or eliminate every risk.

I voluntarily consent to the services identified in this form and acknowledge the importance of following the salon's safety instructions and aftercare recommendations.

This acknowledgment does not release the salon or stylist from liability for negligence, willful misconduct, violations of law, or other liability that cannot legally be waived. It does not limit my right to pursue any claim or remedy available to me under applicable law.

SECTION 10: PERSONAL PROPERTY & LIABILITY

I understand that salon services may involve the use of water, hair color, bleach, toners, chemical treatments, styling products, heat tools, and other professional equipment that may pose a risk of staining, discoloration, or damage to personal belongings.

I acknowledge that I am responsible for securing my personal belongings during my appointment, including but not limited to clothing, handbags, purses, jewelry, eyeglasses, electronic devices, and other valuables. I agree to inform my stylist if I have concerns about protecting any personal item before the service begins.

I understand that hair color, lighteners, chemicals, styling products, and other salon materials may permanently stain or damage clothing, fabrics, accessories, and other personal property. I agree to wear or use protective coverings when recommended and to take reasonable precautions to safeguard my belongings.

To the extent permitted by applicable law, I acknowledge that the salon and stylist are not responsible for loss of or damage to personal property resulting from inherent risks of the services, ordinary staining or product transfer, or my failure to follow reasonable protective instructions, except to the extent such loss or damage is caused by the salon's or stylist's negligence, willful misconduct, or other conduct for which liability cannot legally be excluded.

I agree to notify the salon promptly if I become aware of any damage to my personal property during my appointment so that the circumstances can be documented and reviewed.

Nothing in this section waives or limits any rights or remedies that cannot legally be waived under applicable law.

SECTION 11: CLIENT ACKNOWLEDGMENT

I confirm that:

• I have read and understood this consent and waiver form.

• I have provided accurate information about my hair history and relevant allergies or sensitivities.

• I understand the potential risks and limitations of chemical services.

• I have had the opportunity to ask questions before my service.

• I understand the salon's no-refund policy for completed services, subject to applicable law.

• I voluntarily consent to the services discussed with my stylist.
